Hiring a Caregiver in Taiwan in 2026: Real Costs, and When Only a Licensed Nurse Is Safe

If you are reading this from overseas, the situation is probably familiar: a parent in Taiwan is about to be discharged from hospital, a relative sends you three screenshots of caregiver quotes, and the prices range from NT$280 an hour to NT$12,600 a day. None of it is explained. Here is what those numbers actually mean in 2026, and how to choose without being in the room.
What home care in Taiwan actually costs in 2026
- Caregiver platforms: roughly NT$280–410 per hour with a 3-hour minimum, or NT$2,000–2,700 per shift, plus a platform fee of around NT$100 a day. The workers are care attendants (照服員) with basic training, not nurses.
- Live-in foreign caregivers: NT$22,000–35,000 a month plus room, board, and insurance. The cheapest long-term option, but the application process takes months — it cannot cover a discharge happening this week.
- Private duty nurses: licensed RNs with hospital clinical experience, working one-on-one. At Alma the pricing is fixed: NT$6,000 for a day shift (08:00–20:00), NT$6,600 for nights, NT$12,600 for 24-hour care with two nurses rotating. No platform fees.
- Family doing it themselves: free on paper. In practice it costs lost income, lost sleep, and often the caregiver's own health. For families abroad, it usually means one sibling in Taiwan carrying everything — a cost that shows up later as burnout and resentment.
Caregiver vs. nurse: the difference that matters
Most people Taiwan calls 看護 are care attendants — trained in roughly 90 hours to help with bathing, feeding, mobility, and companionship. A 特別護士 (private duty nurse) is a nationally licensed RN, legally allowed to perform physical assessments, suctioning, tracheostomy care, and wound dressing. A care attendant is not, and a good one will tell you so.
That is where the price gap comes from. The difference between a NT$2,500 shift and a NT$6,000 shift is not friendliness — it is what the person can legally do, and whether they can recognize a problem before it becomes an emergency room visit.
When the cheaper option is genuinely fine
To be fair to the cheaper options: if your parent is alert, stable, has no tubes or wounds, and mainly needs help with daily living and company, a care attendant is the right call. Paying double for a nurse adds nothing.
An RN becomes the only safe choice when any of these apply: an NG tube, catheter, or tracheostomy; recent surgery with a wound that needs monitoring and dressing changes; dementia combined with swallowing difficulty; or an unstable condition where someone needs to judge, at 3 a.m., whether this is a hospital trip. We regularly take over cases where a family started with a cheaper caregiver and switched to a private nurse after a dislodged tube or an infected wound. The savings were spent twice over.
Three traps to check from abroad
- Hidden fees. Platform fees, matching fees, and holiday surcharges routinely surprise families. Insist on a written quote that states every surcharge before you commit.
- No-shows. Ask directly: if the caregiver cancels the night before, what happens? If there is no backup plan, the low price is not a real price.
- Unverifiable credentials. "Experienced" is not a credential. Ask to see the license or training certificate. A legitimate provider will not be offended.
This is, candidly, why we built Alma the way we did. Every caregiver we assign is a licensed RN with hospital clinical experience, pricing is fixed with holiday surcharges stated in the written quote, and we are set up for families coordinating from overseas: you arrange everything over LINE (@205tyguj) or email (hello@caredbyalma.com), we reply within an hour, and you receive regular care updates so you are not depending on second-hand reports. We cover Taipei, New Taipei, Taichung, Kaohsiung, and other areas, from discharge-day post-surgery care to 24-hour care and vacation cover when a live-in caregiver takes leave.
FAQ
How much does a caregiver cost per day in Taiwan in 2026?
Care attendants from platforms run about NT$2,000–2,700 per shift plus platform fees. Licensed private nurses cost NT$6,000 for a day shift and NT$6,600 for a night shift; 24-hour RN care is NT$12,600 with two nurses rotating. Expect holiday surcharges on major festivals.
Can I arrange care for my parent from outside Taiwan?
Yes. Alma handles the entire arrangement remotely over LINE or email, replies within an hour, and sends regular updates on your parent's condition, so you can manage care across time zones without flying back.
When is a licensed nurse necessary instead of a regular caregiver?
Whenever medical procedures are involved: NG tube feeding, suctioning, tracheostomy care, post-surgical wound care, or an unstable condition needing clinical judgment. Care attendants are not licensed for these tasks; an RN is.
Can care start on the day of hospital discharge?
Yes. Alma arranges post-surgery care starting from discharge day. Contact us as soon as a discharge date is set so the nurse can be briefed on the case before your parent leaves the hospital.
